Supporting carers’ navigation of older adults’ care transitions to the community carers perspectives

Discharge from hospital can be challenging for older adults and for their carers. This research project aims to identify carers’ needs in supporting older adults who are discharged from hospital to home.

Study description/purpose

The Monash University is seeking carers (family members, friends, neighbours) of older adults living in the community to take part in a survey for a research project. Discharge from hospital can be challenging for older adults and for their carers and the research project aims to identify carers’ needs in supporting older adults who are discharged from hospital to home.

Results from the anonymous survey will help to develop resources to improve support of carers during hospital discharge.

What’s involved

A survey that is available online or in hardcopy format. It and it will take about 25 minutes to complete.

Recruitment criteria

All carers who have

  • Been supporting an older adult living in the community with chronic health difficulties, and
  • Experience with discharge from hospital to home following admission for a physical health problem of the older adult that they support at any time after 1st January 2020
